Prepare the mailpiece for a USPS prohibitory-order application
This checklist is for the mailer-specific process described in USPS Form 1500, where the advertised matter meets the form’s stated criterion in the addressee’s judgment. It is not a general way to block any sender. After reviewing that scope, preserve the full original advertisement and its opened envelope or wrapper for the application.
Put the application packet together
- Keep every page of the advertisement and the envelope or wrapper that held it. Do not discard any part.
- Confirm that the form’s mailer-specific criterion applies: the advertised goods or services are, in your judgment, sexually provocative or erotically arousing. If unsure about procedure, ask USPS before completing the application.
- Complete and sign the form as its instructions direct, and note the date.
- Write down a reference for your own records: the mailer name as printed and the date the piece arrived. The originals go only to USPS.
- Choose the Post Office where you will submit the packet, and list any questions for the clerk.
After you submit
USPS decides whether to accept the application. After processing, it sends the effective date and instructions for reporting mail. The order does not physically block mail. Keep the effective date and reporting instructions with your reference notes.
